Elevated Community Health Honored with Three Major Awards
We are proud to share that Elevated Community Health has recently received three significant honors that reflect our team’s commitment to our community and our patients.

Non-Profit Program of the Year | Summit Chamber Business Excellence Awards
We are beyond honored to announce that Elevated Community Health has been named Non-Profit Program of the Year at the Summit Chamber Business Excellence Awards. Judged on community impact, organizational culture, and public presence, this recognition belongs to every single person who makes our work possible.
This is the first time we have received this honor, which makes it even more meaningful. With fifteen organizations nominated and six finalists selected, it is a powerful reflection of the impact we are making together in Summit County and beyond.
This achievement belongs to our entire organization. It represents the dedication, compassion, and professionalism of our staff, as well as the leadership and support of our Board of Directors. Together, we continue to advance our mission and deliver high-quality, compassionate care to those we serve. We are incredibly proud of this team!
Women’s Wellness Connection Quality Award | Colorado Department of Public Health and Environment
We are also thrilled to share that the Colorado Department of Public Health and Environment (CDPHE) has selected Elevated Community Health as one of three recipients of the Shirley Hass Schuett Quality Awards for the Cancer Prevention and Early Detection Program, also known as Women’s Wellness Connection (WWC).
Our team was recognized as a top performer in four program performance indicators: rarely or never screened for cervical cancer; severe abnormal cervical screenings with complete follow-up; abnormal breast screenings with complete follow-up; and time from screening to diagnosis for abnormal breast screening results.
We were also recognized as most improved in three additional areas: rarely or never screened for cervical cancer; abnormal cervical screenings with complete follow-up; and data entry timeliness.
This recognition is a direct reflection of the outstanding contributions our staff make every day to the health of women across Colorado.
2026 Platinum Seal of Transparency | Candid
Finally, we are pleased to share that Elevated Community Health has been awarded the 2026 Platinum Seal of Transparency from Candid, the largest database of U.S. nonprofits.
The Platinum Seal is the highest level of recognition available through GuideStar and reflects our strong commitment to sharing comprehensive organizational information with donors, funders, and community partners. Each year, we update our Candid profile with our 990s, board information, and key organizational data to ensure transparency and accountability remain at the center of how we operate.
We are grateful for each of these honors and for the community that continues to inspire everything we do. To learn more about how you can get involved with our organization, visit https://staging.elevatedch.org/get-involved/
