ECH Behavioral Health Services

At Elevated Community Health, our philosophy is rooted in Person-Centered Therapy. We emphasize the therapeutic relationship and the active role of the client in their therapeutic process. This philosophy ensures that our clients receive personalized, compassionate care that supports them in elevating their own mental health and wellness path. Our approach includes:

  • Collaborative Care Planning: Clients identify their current struggles and potential goals. Together with the therapist, they develop a care plan tailored to their needs, including both short-term and long-term goals, strategies, and therapeutic techniques.
  • Integrated Care: Our approach is integrated with the client’s primary care provider to ensure we are treating the whole person, addressing one’s physical, mental health needs, and any factors that influence their overall well-being.

Our licensed behavioral health team consists of professional counselors, clinical social workers, and addictions counselors who work with patients to enhance their social, emotional and behavioral well-being. Our counselors also work closely with medical providers to ensure treatment of the whole person.

We strive to empower patients to make healthy life choices and learn skills to cope with medical conditions, big changes, and difficult life issues. All therapists are trained in evidence-based treatment modalities to address areas like trauma, severe emotional dysregulation, sleep issues, and several other common psychosocial disorders.

All behavioral health services provided through Elevated Community Health are confidential. Behavioral health services might be short term, solution-focused services for immediate needs, or may require longer-term engagement for more severe and long-standing issues. Questions about treatment and duration can be addressed to your provider at the time of service.

Behavioral Health Areas of Expertise

Traditional Person-Centered Outpatient Therapy

At Elevated Community Health, our Behavioral Health Consultation (BHC) Service offers short-term, solution-focused support as part of your primary care experience. Integrated directly with your Primary Care Provider (PCP), a Behavioral Health Consultant helps you explore how your mind, body, and behaviors connect, providing practical strategies to improve your overall wellness. BHCs can assist with managing chronic conditions like diabetes or chronic pain, making lifestyle changes such as quitting smoking or exercising more, and addressing emotional challenges like anxiety, stress, or grief. Available to all Family Health Center patients, visits are about 30 minutes, emphasizing practical, whole-person care without a separate referral or mental health record. This is not traditional therapy but a collaborative, primary care–based service designed to help you navigate life and health challenges; if longer-term support is needed, your BHC and PCP can connect you to specialty care. SBHC Therapy Services

Elevated Community Health provides behavioral health services directly through our School-Based Health Centers (SBHCs), making it easy for students, staff, and families in the Summit, Lake, and Park RE-2 School Districts to access care without leaving campus. These centers offer confidential, compassionate support for emotional, behavioral, and mental health challenges that impact well-being and academic success. To get started, contact your school’s front office, counselor, or SBHC team, complete registration and consent forms, schedule an intake appointment, and begin care—sessions take place during the school day in a private space at school. We accept private insurance, Medicaid, CHP+, and Elevated Community Health Care Cards, with sliding fee options for uninsured patients. Our behavioral health team helps with anxiety, depression, trauma, stress, relationship issues, and more, focusing on supporting the whole student. Integrated Primary Care Behavioral Health

This is not traditional therapy—it’s an integrated, whole-person service that helps you navigate life and health challenges in collaboration with your PCP. If longer-term care is needed, your team can connect you to specialty therapy or psychiatric services. School-Based Behavioral Health

Behavioral Health services for children and adolescents are available at every school in Summit, Lake, and Park Counties. Our School-Based Health Centers provide youth-focused counseling services on school grounds during the school day, increasing access to support while minimizing time away from school or work.
Services are available exclusively to students enrolled in the school and their immediate family members.
