The Elevated Essentials Primary Care membership provides affordable, transparent, and predictable access to high-quality primary care for you and your family.

  • Up to 4 Visits Annually (If you use all the included visits in your plan, each additional visit is $70)

Comprehensive Care for Your Baby’s First Years

The Elevated Essentials Baby Plan is designed to provide infants and toddlers (0–36 months) with the essential care they need to grow and thrive. This membership offers frequent wellness visits, preventive screenings, and early dental care at a predictable monthly cost.

  • Vaccines are provided at no cost, with only a $21 administration fee per vaccine as required by law

Is this health insurance?

  • No — this membership plan is not health insurance.
  • It is a membership program for Primary Care, Dental, and Behavioral Health services at predictable rates.
  • Members pay a prepayment fee for a set number of visits and discounts on additional care.
  • It only covers services provided at ECH.
  • It does not cover emergency care or services received outside of ECH.
  • It does not meet Affordable Care Act insurance requirements.

Can I enroll only my children in a membership plan, or do I also need to enroll as their parent/guardian?

  • Yes — you can enroll only your children.
  • Parents and guardians are not required to enroll.
  • Each membership needs a designated Plan Holder (primary contact and person responsible for payment).
  • The Plan Holder does not need to be an enrolled member.
  • Families can enroll children in any service line without adding adult coverage.

Who counts as a member of my household?

  • Your household includes anyone who lives with you and shares living expenses (spouse/partner, children, dependents, etc.).
  • ECH considers household income and living arrangements to determine eligibility and pricing.

Do I need to enroll in all three service lines—Primary Care, Dental, and Behavioral Health?

  • No — you can choose the service lines that best meet your needs.
  • Each service line has its own membership plan.
  • You can enroll in one, two, or all three.

I have health insurance; can I still enroll in Elevated Essentials to compliment my plan?

  • No — ECH cannot enroll individuals with health insurance in Elevated Essentials Primary Care at this time due to insurance contracts.
  • ECH is working on solutions to offer this option in the future.
  • If your plan does not cover behavioral health services, you may still be eligible for Behavioral Health Elevated Essentials.

I have health insurance, but my plan does not cover behavioral health services. Can I enroll in the Behavioral Health Elevated Essentials plan to complement my insurance?

  • Yes — if your plan does not include behavioral health coverage, you can enroll in the Behavioral Health plan.
  • ECH must verify you do not have existing behavioral health benefits as part of enrollment approval.

I have Medicare. Can I enroll in Elevated Essentials?

  • If you have Medicare Part A and Part B, you can enroll in Elevated Essentials primary care and/or behavioral health services.
  • If your Medicare plan does not include dental coverage, you can enroll for dental benefits.
  • If you have Medicare Advantage, you cannot enroll in Elevated Essentials for primary care or behavioral health.
  • You may enroll in the dental service line if your Medicare Advantage plan does not include dental coverage.

How do I know if I qualify for the Care Card and if I should enroll in that program instead?

  • You qualify if your household income is at or below 200% of the Federal Poverty Level (FPL).
  • Maximum income limits by household size:
  • 1 person: $31,300
  • 2 people: $42,300
  • 3 people: $53,300
  • 4 people: $64,300
  • 5 people: $75,300
  • 6 people: $86,300

Is there a maximum income to enroll?

  • No — there is no maximum income limit to join.

Do I need to pay in advance to enroll?

  • Yes — plans require payment upfront for 3, 6, or 12 months.
  • 6-month prepay: 10% discount on Primary Care and/or Behavioral Health.
  • 12-month prepay: 25% discount on any/all of the three service lines.

Are concierge services included in this plan?

  • No — concierge services are not included.
  • The plan focuses on essential services at affordable rates.
  • Visits are scheduled; there is no 24/7 personal access to providers.

Can I sign up at any time, or only during specific periods?

  • You can enroll at any time.
  • There are no limited enrollment periods or life-change requirements.

Can I cancel my membership at any time?

  • Yes — you can cancel at any time.
  • Coverage remains active through the end of your prepaid term.
  • Fees are not refunded or prorated.

Can I use my HSA to pay for my membership plan?

  • Yes — you can use your HSA to pay your membership fees.
  • This can be a tax-advantaged way to access Primary Care, Dental, and Behavioral Health services.

Where can I use my Elevated Essentials membership?

  • At any ECH location across Summit, Lake, and Park counties.
  • Including school-based health centers.

Why does enrollment take up to 10 days to process?

  • ECH reviews eligibility, processes payment, and sets up your membership.
  • ECH may confirm whether you qualify for a more affordable program (like Care Card).
  • After approval, you’ll receive an email with your virtual ID card and plan details.
