Elevated Community Health is a nonprofit health center, and every donation helps make care possible for our community. Your gift goes directly toward providing primary care, dental, behavioral health, and other essential services for patients who need accessible, compassionate care close to home.
Make a DonationSupport comes in many forms! Please consider these other ways to give. Other Ways to Donate
If you have a DAF, you can recommend a grant to the ECH. You can also include ECH as a partial or full beneficiary of your DAF (EIN #: 20-1139635).
Please ask your DAF representative to include your name and address so we can acknowledge your generosity and share with you the impact your gift makes.
Maximize your impact by making a gift of appreciated securities – like stocks, bonds, ETFs and mutual funds – and avoid capital gains taxes. And if you itemize, it may also provide you with a tax deduction.
To donate publicly traded securities, follow these two easy steps:
Step One
Provide your broker with the following information to initiate your donation.
• Elevated Community Health; EIN#: 20-1139635
• DTC #: 0226, Account #: B9F664857
Reference line: please provide us with the donor’s name
Summit Comm. Care Clinic, Inc. DBA: Elevated Community Health
Step Two
To ensure a prompt tax-deductible receipt for your gift, please share with us the donor name, stock symbol, number of shares, and approximate donation value via email at development@staging.elevatedch.org, or by calling 773-706-4986.
You can also reach ECH’s broker directly:
Tel: 970-668-4040

If you or someone you love are 70 ½ or older, you can use your Qualified Charitable Distribution (QCD), up to $111,000 per person in 2026. And if you’re 73 or older, you can take your Required Minimum Distribution (RMD) to support Elevated Community Health’s ongoing work.
To arrange a QCD or RMD from your IRA, follow these 3 easy steps:
- Contact the financial institution where your IRA is held and instruct them to send all or a portion of your RMD to Elevated Community Health, federal tax #: 20-1139635
- Ask that they send the check directly to Elevated Community Health: PO Box 4337 Frisco, CO 80443, federal tax ID # 20-1139635. It only counts as a QCD or RMD if the check goes from the financial institution directly to a charity.
- Let us know, so we can properly acknowledge your gift, either via email: development@staging.elevatedch.org, or phone: 773-706-4986
No matter what you decide, you can always designate Elevated Community Health as the beneficiary of all or a percentage of your IRA and it will pass to us tax-free after your lifetime.
If you have already, or are considering, including Elevated Community Health in your will, estate or as a beneficiary of your IRA or retirement funds, we warmly welcome you as a member of our Legacy Society.
