Elevated Community Health is a nonprofit health center, and every donation helps make care possible for our community. Your gift goes directly toward providing primary care, dental, behavioral health, and other essential services for patients who need accessible, compassionate care close to home.

Donor Advised Fund (DAF)

If you have a DAF, you can recommend a grant to the ECH. You can also include ECH as a partial or full beneficiary of your DAF (EIN #: 20-1139635).

Please ask your DAF representative to include your name and address so we can acknowledge your generosity and share with you the impact your gift makes.

Appreciated Stock

Maximize your impact by making a gift of appreciated securities – like stocks, bonds, ETFs and mutual funds – and avoid capital gains taxes. And if you itemize, it may also provide you with a tax deduction.

IRA Giving

If you or someone you love are 70 ½ or older, you can use your Qualified Charitable Distribution (QCD), up to $111,000 per person in 2026. And if you’re 73 or older, you can take your Required Minimum Distribution (RMD) to support Elevated Community Health’s ongoing work. 

To arrange a QCD or RMD from your IRA, follow these 3 easy steps:

  1. Contact the financial institution where your IRA is held and instruct them to send all or a portion of your RMD to Elevated Community Health, federal tax #: 20-1139635
  2. Ask that they send the check directly to Elevated Community Health: PO Box 4337 Frisco, CO 80443, federal tax ID # 20-1139635. It only counts as a QCD or RMD if the check goes from the financial institution directly to a charity.

No matter what you decide, you can always designate Elevated Community Health as the beneficiary of all or a percentage of your IRA and it will pass to us tax-free after your lifetime.

Decide what percentage (1 to 100) you would like us to receive and name us, along with the percentage you chose, on the beneficiary form.   2. Return the completed form to your plan administrator, insurance company, bank or financial institution.

In 2024, 49% of our patients were uninsured. A sliding fee scale based on family size and income is available for patients who pay for health services out of pocket. More than 86¢ of every dollar you generously contribute directly supports patient services. By donating to Elevated Community Health, you are personally ensuring health care services are available to all, regardless of ability to pay, and investing in the healthy future of our community.

See who Funds the Elevated Community Health

Funding for Elevated Community Health’s operations comes from a variety of sources, including:

 

Local, State, and Federal Government 

– Summit County Government supports Elevated Community Health by providing in-kind office space in the Medical Office Building in Frisco, as well as grant funding for programs such as nicotine cessation and behavioral health services. The State of Colorado also contributes through tobacco tax revenue and competitive grants that help fund services like family planning and cancer screenings. As a Federally Qualified Health Center (FQHC), we receive federal funding to support a range of programs, including our sliding fee scale (Care Card) program.
 

Patient fees and Insurance Reimbursement 

– Elevated Community Health collects payment based on the healthcare services provided. This may include Care Card co-pays, insurance reimbursements, and direct payments from patients.

 
It is worth noting for your knowledge, not all patients receive services on a discount, it is only if their income is at or below 200% of the Federal Poverty Level.
 

Foundations 

– We receive funding from a number of private foundations including: The Summit Foundation, Caring for Colorado Foundation, The Colorado Health Foundation, Vail EPICPromise, El Pomar, Rocky Mountain Health Foundation, and Reach out and Read.

– Elevated Community Health has a long as highly reputable reputation for responsible stewardship of charitable gifts. We are honored by the outpouring of generosity from our community and we work hard everyday to make sure that your gifts are used prudently and to deliver our services in the most effective and efficient manner.
