Ways to Contribute

Donor Advised Fund (DAF)

A simple, flexible, and tax-efficient way to give to your favorite charities. A donor-advised fund is like a charitable investment account, for the sole purpose of supporting charitable organizations you care about. You can recommend a grant or recurring grants from a DAF to make an immediate impact or to use as a tool for future charitable gifts. Taxpayer EIN: 20-1139635

Individual Donors

Our donors are critical to our mission of providing care to everyone, regardless of their ability to pay. None of our neighbors or their children should ever have to go without healthcare. Cash, check, credit card or stock contributions may be made at any time to support general operating or designated funds. Planned Giving

Support the ongoing ability of Elevated Community Health to provide critical health services Please consider Elevated Community Health in your estate or legacy plans through a will, trust, retirement plan, life insurance policy, or life-income gift. We can help if you are interested in a way to redirect your taxes to benefit your family as well as your community and our mission. Bequests through wills or trusts, life insurance, or real estate allow future and extended support to Elevated Community Health. Taxpayer EIN: 20-1139635 Bequest A bequest is a gift made through your will or trust which indicates your desire to leave something of value toward Elevated Community Health’s future work. A charitable bequest can be a specific dollar amount or a percentage of the value of your total estate, a gift of stock or life insurance, or even a gift from an IRA or other qualified retirement plan. Because these assets often carry a heavy income tax obligation to your heirs, they can be an attractive source for funding your charitable gifts.

Stocks

Electronic delivery of stock shares is a secure and expedient delivery process available and provides efficient internal control and cost savings. You may also transfer certificates directly. These donations are considered non-cash donations. Please note, the donor’s broker will provide the donor with a tax receipt for the fair market value on the day the shares are transferred. Elevated Community Health will send the donor an acknowledgment letter that describes the gift of stock, including the ticker, the number of shares, and the date of the donation.   1. Provide Elevated Community Health a notification of donor intent by emailing development@staging.elevatedch.org. Regardless of the method chosen to deliver a gift of stock, the donor or the transferring broker should provide the following information for audit and acknowledgment purposes:
    • Donor’s name, complete address/contact information, number of securities transferred and date of transfer.

Volunteer Opportunities at Elevated Community Health

Volunteer – Special Events and Light Office Work

Please email development@staging.elevatedch.org with your interests and contact information. For further information, please call (970) 409-4823. Special Events Upcoming Event: Soup for the Soul, Bread for the Clinic Date (Tentative): Saturday, March 26, 2026 Office Assistance General clerical duties supporting daily operations, including sorting and distributing mail, answering telephone calls, and electronic filing.

Volunteer – Specialty Care Providers, Residents, Internships, Job Shadows

Students, interns, and trainees are encouraged to send their cover letter and introduction letter to careers@staging.elevatedch.org. All interested participants must complete the required paperwork before beginning any clinical work. Please submit your program details and requirements to Human Resources.

Clinical Volunteer – Specialty Care Providers

Specialty care providers donate over 1,000 hours of service annually. Services include:

  • Dermatology
  • Endocrinology
  • Orthopedics
  • Rheumatology

  • Behavioral Health
  • Dietary Counseling
American Psychological Association Internship

Elevated Community Health is a site for the APA-Accredited Pre-Doctoral Internship in Psychology through the Colorado Psychology Internship Consortium (CO-PIC).

Behavioral Health Training

Advanced Practicum & Master’s Internship Program

Training positions are available in both school and in-office settings with supervision by the Behavioral Health Service Line Director. All positions are direct hire and paid.

Rural Rotation for Family Medicine Residents

Elevated Community Health is a rural rotation site through the Colorado Association of Family Medicine Residencies. Residents complete 4-week rotations covering:

  • Pediatrics
  • Chronic Care Management
  • Office-based Procedures

Supervision is provided by the Chief Medical Officer and board-certified Family Medicine physicians.

Dental Assistant Training Program
Training includes a combination of textbook learning, lectures, and hands-on experience in our dental office.
